School Home Support
Gladesmore Community School is excited to be the pilot school for a new initiative to aid primary to secondary transition. This work is supported by an independent education charity called School Home Support.
Family Transition Support Worker, Ann Cockbill, is based at the school and is dedicated, full-time to working in partnership with the Year 7 team as an additional link between home and school in support of children and families. She liaises with parents and carers, staff and external agencies to help address any worries, problems and issues that arise. Her work is wide-ranging and aims to get solutions with families.
Ms Cockbill has an extensive knowledge of voluntary and statutory organisations who work with local families and is able to use this knowledge to signpost parents and carers to access an array of support and complete appropriate referrals.
She offers transition support and advice to families and pupils currently in Year 6 in primary schools and those in Year 7 at Gladesmore. This includes children with diagnosed Special Educational Needs & Disabilities (SEND) and those not formally diagnosed.
